Yeah that’s a postnatal discharge looks like it’s being put into the wrong record. If it’s your GP record you need to contact them and flag asap as it’s a GDPR data breach which is serious. (I’m a GP service manager and deal with patient records) x
I hate my pregnancy notes sometimes. Not the first time I’ve heard of wrong info being put in there, happened to me as well with a different thing
update: my midwife called and told me it was a mistake, she’s told the hospital and IT department to fix it

I’d report that to your midwife. It says you gave birth on 9th October, you look like you’ve been mixed up with another patient which could be a GDPR breach tbh. Also, you want to make sure you don’t have your care stopped cause the system has you as delivered.
