Stalled Miscarriage?

I’m curious if anyone has gone through what I’m going through now. I have been actively bleeding since the 20th like a period. A couple days before that started my OB said she believed this would be another miscarriage as I was measuring behind and the sac looked abnormal. No heartbeat. I went in yesterday to confirm the loss (which they did) and start misoprostal. Well, before the ultrasound yesterday, I felt it officially starting. I bled through a pad in just over an hour and felt a bit (not a lot) of tissue come out in the toilet. After the US and pills though, it significantly slowed down. Bleeding went back to period levels with barely any tissue coming out. I had bad cramping yesterday and almost none today. Other than the bleeding, this miscarriage is identical to my last where I miscarried between 9-10 weeks with baby measuring 6 weeks. But last time I had SIGNIFICANT tissue loss I passed some clots the size of baseballs and golf balls. But this time around nothing even close to that. Am I somehow just needing to pass less tissue or has it stopped somehow? Do you think I’ll end up needing a D&C? I’m worried that nothing is actually coming out other than some blood

I'm so sorry for your loss ❤️ with my missed miscarriage I bled similar to you and I passed tissue on 2 separate occasions before taking the medication, when I then passed a bit more. When I had another scan they confirmed the miscarriage was incomplete. Do you have another scan booked so you can get an idea of what's going on?

@Rowan sadly my next scan isn’t until July 10th to make sure it’s complete. Not really sure what to do at this point :/ especially bc I was supposed to be traveling next week I’m wondering if I need to cancel

I’m really sorry. This is so hard. It sounds like you have more tissue to pass. I would wait the two weeks since you started bleeding, if still bleeding go for US. This happened to me with my MMC. I took the medication, passed a bit of tissue and then continued to bleed off and on for 3 weeks. At my follow up US, they confirmed a lot of retained tissue and did recommend D&C. I didn‘t want to so took 2 more doses of medication, in which I was able to get the rest out over another 2 weeks.

How far along were you this time? You mentioned no heartbeat so I’m guessing there was an embryo and you were expected to be at least 6-7 weeks? If the sac is smaller, possibly there is less to pass. One way I knew I had retained tissue is I continued randomly cramping for weeks after medical management. I took it again and it wasn’t successful so they did a more targeted surgery. Unfortunately you can’t really know until you have another US. When I had a blighted ovum I had only one clot come out and minimal bleeding, but like you when baby measured 6-7 weeks there were golf ball sized clots coming out. If the sac looked abnormal maybe this could explain the difference? Sorry it’s so difficult - it’s a horrible thing to have happen x

@Tarra ugh that’s what I’m afraid of. With my last miscarriage I had the whole thing almost naturally, my baby was already on her way out (literally at the edge of my cervix) when I started the pills. This time they had me start the pills a week earlier than last time but they honestly seem like they’re doing nothing

@Nerida this time around they said the fetal pole measured 6 weeks 5 days and gestational sac measured 5 weeks 4 days. My uterus is like 7.5cm whereas last time, as I mentioned, I miscarried around the same time with the same gestational age but my uterus was 11cm last time so yea I’m not really sure how much to expect. And not sure if the pills are causing the cramping or if I’m cramping cause I didn’t get it all out

Update for everyone: I got another ultrasound today which confirmed the gestational sac is still there, but it’s close to the cervix. But bleeding has slowed significantly I’m only getting blood when I wipe and not on my pads. And no cramps. So now I’m waiting for my doctor to message me back and I have no idea what next steps are 🫠 I’ve tried pushing on the toilet and nothing happens

Sorry to hear! Hopefully another course of pills will do the trick and you won’t need surgery.

I tried misoprostal twice and while it caused a bit of bleeding and cramping, it didn't work and I needed surgery. It's only effective in about 85% of cases sadly

@Robyn it sucks. It worked with my last, but honestly I was about 85-90% of the way there myself so who knows if it really even did anything. I have surgery scheduled for Monday

Hope it goes as well as it can for you. I had to wait 4 weeks for my surgery because I had pneumonia and wasn't well enough for anaesthetic but when I did finally get it, it was so straightforward and I would go that route again if I needed to
