MatB1 Forms

My midwife said she’ll give me by MatB1 form at our 28 week appt - which won’t be till December. Is anyone getting there’s sooner? Seems short notice as my employer can’t process paperwork till they have that (NHS).

I phoned up the community midwife hub and they got it ready for me to collect as it would have been after the 15 weeks notice we legally have to give our employer!

They can give it to you anytime after 21 weeks in my trust. I have an appointment this week so getting it then but they should be giving you this before 25th week as your employer needs that on file by that point. I would contact them and ask for it earlier. Just need to go pick it up x

I chased mine from my GP once the GP has signed it they send it off and you get a copy

I called by GP after my 20 week scan and asked for it stating I couldn’t wait until 25+ weeks as my employer is asking for it. They spoke to my midwife and I collected it a couple of days later from them

Yeah i got mine a few weeks back as they weren’t going to give me it until 25+1 and that was past the cut off date the government says to give to the employer. I just phoned the midwife unit and they got it ready for me to collect

I got mine after my 20w scan with no issues. I'm greater glasgow & Clyde health board. But I know some wait until after the viability window to issue x

I got mine at 21 weeks after asking for it

I had to go to the hospital at 21 weeks for an OB appt. which was after my scan and just saw the admin midwife office to sort. My next midwife appt. isn’t until 28 weeks so would’ve been waiting till then too.m but actually would’ve had to do the same thing and see the admin office rather than whoever was doing the appt. as that’s how it works at my trust. You need to inform your employer 15 weeks prior to the due date you are pregnant however this doesn’t mean needing the Matb1 form. If they want proof of pregnancy there are other ways to do this with scans etc. Some employers can be difficult about it but they shouldn’t be and if you say that’s the next time you see the midwife to collect then it should be ok. As long as you’ve informed them in writing and would just keep them updated on getting this form.

My next appointment isn’t until 28 weeks so my midwife said to contact the gp to pass on a message to her and she completed it and left it at reception for me to collect. They can give it to you anytime after the 20 week scan.

I got mine the day of my 20 week scan I called my midwife and cos she was at the hospital she gave it to me there and then maybe ask to see them or call the community midwife

NHS as an employer normally ask for the MATB1 form to go off with maternity leave application form as evidence on or in your 25 week at latest. Try calling your midwife or GP and see if you can get it earlier.
