Toilet training

Hey. My little girl came out of nappies around 7/8 months ago and has had very little pee accidents since but pooing on the toilet is a really big issue. She has been pooping her pants for months and seems to have a fear of pooping on the toilet. She holds it in until we have had to have hospital trips it’s been so bad. Crying for her pants to go back on so she can poo. She takes regular laxatives to help keep them from being sore as I thing that’s what maybe started this issue. Does any have any advice or had a similar experience? I am not getting much support from professionals and it’s taking its toll on us all now especially my little girl.
