Mommy labor nurse has a birth plan template (on Instagram or search her url) and mamma natural also had a great one that was not so much open ended questions but multiple Choice that I found to be really helpful.

There are a lot of templates I’ve found online that are a great start. They have different designs and options that can help you get started!

I was given a form to fill out at my midwife to bring with me to the hospital. Just remember it’s a plan, but things don’t always go according to plan. Be open to changes. The birth plan is what you want during the birth. Think about other situations when things go wrong, what would you like done.
