GD & large baby

Diagnosed with GD recently and I had one scan showing large abdominal growth 2 weeks ago. I attended to collect the blood sugar monitoring machine today and then the diabetic midwife pulled me aside and told me that I will need to start insulin or metformin on Monday. I asked why I was not allowed to trial diet control, and she told me that it’s because of my growth scan result. She stated this is in the nice guidance. My fasting levels at diagnosis were fine (4.4) The nice guidance says to recommend medication/insulin immediately to people only where their fasting levels are between 6-6.9 PLUS a large baby on the growth scan. And the guidance says to let people otherwise try diet for 2 weeks. I expressed concerns she was not giving me the 2 week opportunity and she has put in my notes that I was unreasonable. I don’t have any other risk factors listed in the nice guidance. I have a joint appointment with her and the consultant on Monday where she said to expect this to be rediscussed. Any advice? X

It's your decision not there's.go in armed with your NICE guidance. Tell them you don't appreciate being labelled as unreasonable and that you will be attempting to diet control first x

I agree with Alana, its ultimately your decision so to say you’re unreasonable for questioning & trying to understand why, isn’t right. It’s unreasonable on their part actually. If I were you, I would go into the meeting with an open mind but armed with the knowledge you’ve gained through reading the guidelines. Listen to the consultant, ask as many questions as you need and then tell them how you wish to proceed. If you opt to try diet control for 2 weeks, they should do whatever they need to assist you with that and monitor accordingly. If you don’t get a satisfactory outcome, you should file a complaint and ask to see another consultant. They can’t steamroller you into something you don’t agree with, advocate for you & your baby, you got this 💪 x

I agree with you completely, TBH they told me something similar. My fasting came out at 5.5 and post GTT, it was 9.7. They suggested me to try diet for two weeks, in my first week my fasting sugars fluctuated and it was in the range of 5-5.8, met diabetic consultant, doctor and nutritionist after my growth scan. They told me my fasting sugars are really not good and I might have to consider medication soon. I asked them to give me one week time and see if I can manage it, if not I will definitely consider medication.They tried to convince me, but since I insisted and requested for only one week trial, they just gave me.You can ask as many questions as you want and in general the consultants are kind of reasonable.I have spoken for almost 1.5hrs with the consultant and nutritionist before we reached to the agreement. May I know what's the EFW(fetal weight) during your scan ?? That would be one of the deciding factors during these types of negotiation.

What gestation was that at? Mine was 2.7lb at 28+6. It's worth bearing i mind that abdominal circumference is the most inaccurate and that it can vary based on weather baby has a full bladder or not, also!

That's definitely not a big baby...mine weighed at 3.11 lb at my 29+3 last week. So, feel free to talk to them and ask for one week food trial. Infact, it would be hard to say that they would be big babies based on the growth chart plotting. What they did was your first baseline reference and they should be monitoring and can only tell you in the future scans on how the baby is growing based on future weight growth. Unless there is rapid growth from your previous scan, it won't be a big baby.

Just for some perspective also, my fasting was 5.2 and my after GTT was 7.8 . I'd also be questioning why they're asking you to go straight onto medication when they've not even seen what trends are for after meals on your monitering. Your fasting levels are well below so you may well be able to control your after meals with diet. Look on the Gestational diabetes website and Facebook group for meal ideas and nutietikn advice, because quite frankly the NHS advice is shocking and the things they suggested for me to eat really spiked my sugars!

Thank you for your advice. This is my thoughts exactly - they’ve made that decision based on one scan & zero bloods monitoring. She was very reluctant to let me go home as well and was not happy that I didn’t take the insulin there and then. A very poor experience. Thank you - I’m in the GDUK group and had noticed a stark difference with the guidance there and the guidance the nhs gave, a lot of people have said it’s outdated x

Please don’t be scared of meds either. I’ve been on insulin since 10wks and I’d rather have stable blood sugars that be struggling and cause other issues with ketones by not eating enough
